FunctionAbility Rehabilitation Services has an immediate opening for a community-based role of Social Worker (TEER 1: 41300) within the Ottawa, ON region.
As a Social worker you will perform some or all of the following duties:
• Plan, coordinate, manage and implement support packages to help clients deal with difficulties and overcome dependencies who have experienced trauma, traumatic brain injuries, spinal cord, and complex orthopedic injuries; • Interview service users and assess their current condition, needs, strengths and weaknesses; • Address each case as a unit and set tailored measurable goals; • Give advice and therapy to help clients learn coping mechanisms for handling and resolving their personal and social issues; • Monitor and evaluate clients’ progress and modify treatment plans accordingly; • Offer information and counseling on the best course of action during sessions; • You will connect clients with a variety of relevant community-based resources and services. • Maintain accurate records and report on clients’ status; • Attend case conferences and provide evidence in court (if required); • Act as a key-worker and cooperate with multidisciplinary teams; • Create or advise on social policy laws, carry out social research, and support community development; • Refer clients to community services to help them in recovery; • Evaluate the success of social services and counseling.
Qualifications: • A bachelor's degree in social work is required in Newfoundland and Labrador, Nova Scotia, Prince Edward Island, New Brunswick, Quebec, Ontario, Manitoba, Saskatchewan, British Columbia, the Northwest Territories and Nunavut. • Supervised practical experience is usually required. • Successful completion of provincial written and oral examinations may be required. • Registration with a provincial governing body is mandatory to practice as a social worker in Newfoundland and Labrador, Nova Scotia, Prince Edward Island, New Brunswick, Quebec, Ontario, Alberta and British Columbia. • Use of the titles \\\"Social Worker\\\" and \\\"Registered Social Worker\\\" is regulated in all provinces. • Membership in a provincial association of social workers is usually required.
