Social Worker Job

Key Responsibilities
Conduct comprehensive psychosocial assessments to identify patient and family needs.
Develop individualized care and discharge plans in collaboration with the healthcare team.
Provide counseling and crisis intervention to support patients and families coping with illness, hospitalization, or major life changes.
Advocate for patient rights and assist with access to community resources, financial assistance, housing, transportation, and other social services.
Facilitate referrals to outside agencies and coordinate multidisciplinary care as appropriate.
Document all assessments, interventions, and outcomes accurately in the electronic medical record.
Educate patients and families on available programs, benefits, and follow-up care.
Collaborate with physicians, nurses, therapists, and community partners to ensure seamless transitions of care.
Participate in interdisciplinary rounds and case conferences.
Maintain confidentiality and compliance with all regulatory, ethical, and professional standards.
Participate in performance improvement initiatives and professional development activities.

Qualifications
Education: Master’s Degree in Social Work (MSW) required; Bachelor’s in Social Work (BSW) may be acceptable depending on setting.
Licensure: Current state licensure as a Licensed Social Worker (LSW, LMSW, or LCSW depending on state regulations).
Experience:
Minimum of one year of clinical or healthcare-related social work experience preferred.
Experience in hospital, behavioral health, or community-based settings desirable.
Skills and Competencies:
Strong assessment, counseling, and case management skills.
Knowledge of community resources, healthcare systems, and social service programs.
Excellent communication, empathy, and problem-solving abilities.
Ability to work independently and as part of a multidisciplinary team.
