Job Description:
Assessment and Planning (40% of the time):
 Collaborate with the patient, family, and multidisciplinary team (MDT) to gather detailed information about the patient’s situation and functional status, in order to identify individual needsand develop a comprehensive plan of care.
 Identify both present and possible future needs of the patient and family, which may affect the plan of care and the patient’s well-being and physical and mental health.
 Generate assessment reports that include age-specific physical, psychosocial, environmental, financial, and health status expectations.
 Identify specific objectives, goals, and actions based on the findings from the assessment process.
 Provide appropriate counseling services to patients and/or families based on the assessment and consistent with the patient's ongoing medical condition/needs and social servicesupports.

Implementation, Monitoring, and Evaluation (40% of the time):
 Execute specific interventions that will lead to accomplishing the goals and time frames of the shared plan of care. Help patients overcome dependencies, recover from illness, andadjust to life.
 Work effectively with the healthcare team to determine the necessary steps to achieve the plan of care.
 Perform in-depth screening for internal and external community programs for the purpose of educating, navigating, and connecting patients with needed services.
 Obtain sufficient information from all relevant resources in order to determine the effectiveness of the plan of care and services provided.
 Conduct an initial assessment and subsequent reassessments for social services at a minimum of every six months for each ADHC participant and monitor implementation.
 Modify the plan of care, as necessary, in coordination with the healthcare team, family members, and providers.

Other (20% of the time):
 Make referrals to available community resources. Maintain a current roster of community resources.
 Develop and maintain positive working relationships with the patient and family, physicians, healthcare members, community, and payor representatives.
 Appropriately documents the plan of care, outcomes, statistical reporting, logs, and files abiding by departmental, legal, and regulatory requirements.
 Attend and participate in all the MDT meetings.
Job Requirements:
Minimum Requirements:

 Master’s degree in Social Work, Gerontology, or related field;
 Proficient in MS Word, Excel, and Outlook.
 Strong knowledge of psychosocial assessment tools, care planning, and case management.
 Proficient in maintaining accurate and organized clinical documentation.
 Strong communication, interpersonal, and organizational skills
