Social and Community Service Manager Job

Assist and advocate for clients to apply for and enroll in government programs (Medicare, Medicaid, SNAP, SSI, MSPs) and health insurance plans including Medicare Advantage.
Connect clients to community resources for housing, financial literacy, and legal/estate planning needs; refer to licensed professionals when appropriate.
Work with government agencies and community organizations to stay current on available benefits, policies, and regulations.
Manage a team supporting MAPD clients across all health and insurance needs, including chronic disease monitoring, hospitalizations, and prescriptions.
Track client cases and follow through to ensure under-privileged individuals and families receive the support they need.
Train and mentor team members on social service delivery and compliance with program guidelines.
Prepare program evaluations documenting outcomes and areas for improvement.
Supervise Social Workers, all other.

Master’s degree in Social Work, Healthcare Administration or related fields. Must have Social Worker License, Senior Health Insurance Program (SHIP) Certified Counselor. Must have Knowledge of Social Work Practice with Individuals and Families in Gerontology, with a focus on aging care through a multidisciplinary understanding of the biological, physical, psychological, sociological, spiritual, and economic aspects of aging.
