Hours per Pay Period: 1.00 = 80 hours every two weeks (Non-Exempt)
Starting Wage DOE: $35.34 - 44.18

The incumbent in this position acts as an advocate for patients serving as the primary point of contact and is responsible for coordinating the care and services of the identified neurology patient populations for an extended period of time across the continuum of care for both inpatient and outpatient services; promoting effective resource utilization; assuming a leadership role with the multidisciplinary team to achieve an optimal clinical outcome.

This role requires the candidate to assist in a variety of clinical and operational aspects of workflow within the Neurology Department. These clinical duties include assisting provider in performing image guided (Stereotactic, Ultrasound, etc.). Knowledge and competency in sterile procedures necessary. Operational aspects include but are not limited to schedule coordination, follow-up communication with patients, outside care facilities, and referring providers, documentation and record maintenance for necessary records for accreditation and continued care needs.

*Coordination of Care:
*Navigates the patient through the entire care continuum, identifying and eliminating barriers such as scheduling, testing delays, or result reporting.
*Collaborates closely with the interdisciplinary care team to ensure timely appointments, results, referrals, and follow-up care.
*Works to optimize patient flow and ensure all aspects of care are delivered in a timely, effective manner.
* Clinical and Care Coordination:
*Evidence-Based Care: Assists in the development and adherence to clinical guidelines and best practices, aiming for optimal clinical outcomes.
*Ensures adherence to care plans and promotes patient compliance through regular follow-up.
*Collaborates with medical teams to create and evaluate clinical pathways, auditing clinical practice as needed.
*Patient Follow-Up: Facilitates ongoing follow-up for both clinical and non-clinical aspects of patient care (e.g., medication reconciliation, critical values, core measures).
*Clinical Decision Support: Ensures all patient needs are addressed in both inpatient and outpatient settings, coordinating care between all involved teams.
* Educational and Resource Support:
*Patient and Family Education: Provides and coordinates educational resources for patients and their families related to their condition, treatment options, and available support systems.
*Collaborative Care Resources: Works with other healthcare professionals to ensure access to necessary services, including financial, social services, and complementary care.
*Community Engagement: Engages with community and healthcare providers to support patient care needs, ensuring access to community resources and services.
*Caregiver Support: Identifies patient and family needs and assists with appropriate referrals to ensure comprehensive care, i ncluding support for caregivers.
* Quality Improvement & Compliance:
*Data Management and Reporting: Collects and maintains accurate data as required by department guidelines to monitor quality metrics and patient outcomes.
*Quality Assurance: Participates in quality improvement initiatives to monitor and enhance the quality of patient services through audits, reviews, and feedback.
*Regulatory Adherence: Ensures compliance with all applicable laws, regulations, and standards, maintaining a focus on patient safety and quality of care.
*Continuous Learning: Complies with all mandatory education, including the annual Comprehensive Stroke Program education requirements.

Minimum Qualifications
Education
*Bachelor's of Science in Nursing, required

Experience
* 2 years of clinical practice experience, required


* 5 years of experience in Neurosciences, preferred
