Job Requirements:

- Valid Registered Nurse (RN) license in the State of Illinois (Required)
- Minimum 1 year of experience as a licensed RN in a hospital setting(Med-Surg, Telemetry, ER, or ICU experience preferred)
- Minimum 2 years of home health or Home Health Care experience
- Valid driver’s license
- Comfortable using EMR systems (Epic and TruChart preferred)
- High School Diploma or GED (Required)
