Job Description:
• Assist residents with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, mobility, and meal assistance according to individual service plans.
• Monitor and document changes in residents’ physical, emotional, or behavioral conditions; report concerns and recommend adjustments in care levels as needed.
• Prepare, cook, and serve meals to residents while monitoring and recording any changes in eating habits or dietary needs.
• Administer and/or supervise self-administered medications following company policies and regulatory guidelines.
• Maintain cleanliness and organization of resident rooms, common areas, and facility spaces.
• Assist residents with personal laundry and linen changes as required.
• Inspect, clean, and monitor equipment and supplies to ensure safety and proper functioning.
• Schedule appointments and coordinate appropriate referrals for follow-up medical care.
• Document health updates and communicate changes in resident conditions to the on-call nurse and primary care physicians.
• Accompany residents on social outings and activities, ensuring safety and engagement.
• Communicate with residents’ families and provide regular status updates in a professional and compassionate manner.

• Patient, tactful, dependable, and able to work with residents at varying cognitive or maturity levels.
• Must not pose a direct threat to the health or safety of others in the workplace.
• Must be able to pass a criminal background check.
