Your Role
Provide monthly calls and support to patients enrolled in the CCM program.
Utilize your nursing skills to educate patients and their families.
Monitor patients progress and work with the physicians to ensure quality patient care.
Qualifications
Active CMA Certification or Registration (nationally recognized)
Experience in geriatrics or primary care
Ability to sit and talk on the phone for long periods of time
System: We require you to have an i5 or newer Windows processor and to have Windows Pro (not Home). This can be a laptop or desktop.
The Pay
The CMA pay ranges from $17.35 to $19.27 per hour
The pay is a base pay of $15.00/hr plus a bonus of $1.00 per call. On average, our Care Coordinators complete between 16-21 calls per day. High performers may be eligible for a bonus up to $1000 per quarter.
