Rehab Program coordinator Job

Rehab Program coordinator is needed to perform the following duties:

• Planning, developing, organizing, implementing and directing therapy services as well as its programs and activities
• Ensure that therapy plan is those indicated in the care plan for resident. Advice superior of any need to adjust therapy plan to match or support stated care plan goals.
• Assist in developing and maintaining therapy standard.
• Assist in developing, implementing and coordinating policies and procedure, resident care plans, therapy procedure manuals and job descriptions, etc.
• Participate in discharge planning, development, and implementation, of resident care plans, resident assessments as necessary.
• Maintain a current file of resident treatment, progress notes as requires and ensure resident records and not lost/ mishandled.
• Ensure business office receives charges for therapy rendered to the resident
• Maintain an adequate liaison with families and resident.
• Interview resident, family members as necessary
• Agree not to disclose assigned use ID and password for accessing resident/ facility information and promptly report suspect or known violations if such disclosure to administer.
• Report any known or suspect unauthorized attempts to access facility’s information system.
• Agree not to disclose resident’s protected health information and promptly report suspect or known violation of such disclosure to the administrator. Other duties as deemed necessary and appropriate or as may be directed by director of Rehabilitation.
• Management of quality care and staff management.
• Budget and planning Functions including repot suspected or known incidence of fraud relative to false billings, cost reports, kickbacks, etc.
• Understanding of various aspects of health and disease list the methods of health administration, health education and disease preventive measures.
• Maintaining HIPPA and confidentiality.
• Serve on, participate in and/or attend various committees of the facility such as Restrain committee, fall committee, Risk management committee as required or delegated
• Provide written and/or oral report of therapy programs and activities as require.
• Evaluate and recommendation from established committees as they may pertain to therapy service. Participate and represent Rehabilitation at weekly Utilization Review meeting as assigned by Director of Rehabilitation
• Report meeting minutes to Rehabilitation team and Director of Rehabilitation.
• In service and staff meeting to educate and learn importance of use of PPE to safeguard rehab staff, patient, family member or companion of the patient.
• Maintain an excellent working relationship with rehab. Staff, medical profession and other health related facilities and organizations
• Create and maintain an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the department
• Assist in the training of competent rehabilitation personnel as require and Coordinate rehabilitation department with another department.
• Serve as clinical instructor to therapy student as assigned by Director of Rehabilitation.
• Report occupational exposure to blood, blood fluids, infection materials and hazardous chemical to Director of rehabilitation.
• Evaluate, initiate restorative program for residents in conjunction with resident’s plan of care
• Brief resident of procedure involved in rehabilitation, explain role of various therapy and needs of necessary equipment
• Assist in transporting residents to and from rehabilitation room/area.
• Perform therapy in resident’s rooms as necessary.
• Administer therapy plan in accordance with establish policies and procedure
• Encourage resident to perform and/or participate in ADL function
• Assist, direct therapist in administration of rehabilitation plan
• Assist residents in ambulatory training
• Ensure that all therapy notes are informative and descriptive of the care provided and of the resident’s response to the care.
• Administration of proper therapy and applying agent such as diathermy, ultrasound, infrared, massage, therapeutic exercise etc.
• Educate and train patient, family member and nursing staff when required.
• Ensure that therapy procedure is those indicated in the care plan for resident. Advise department supervisor of any need to adjust therapy plan to match or support stated care plan goals.
• Participate in resident assessment care plan and activities as necessary
• Assist, as necessary, in developing rehabilitation plan for individual residents in coordination and conjunction with the resident assessments.
• Ensure that care plans indicate therapy when such therapy is ordered.
• Provide therapy/ supervise implementation treatment in accordance with resident’s plan of care. Ensure therapy treatment in accordance with the president’s plan of care
• Follow safety standards established for rehabilitation department.
• established departmental policies and procedure at tall time, including appropriate dress code.
• Be alert for resident safety during any/all therapy
• Comply with the facility’s established safety precautions at tall times
• Ensure safety and comfort of resident
• Ensure therapy area is maintain in a clean and safe manner for resident comfort and convenience by assuring that necessary equipment and supplies are maintain to perform such duties/service
• Report missing/illegal labels or MSDS’s to your superior.
• Maintain equipment and supply needs of the department
• Utilize all department equipment and supplies in an efficient manner that promotes desired therapy outcomes and minimizes waste Make sure equipment’s is being operated in safe manner.

Bachelor’s Degree is required in Physical Therapy or Physiotherapy or Rehabilitation Counselling
