Location: 1010 PLYMOUTH RD STE D, York, PA – 17402 Onsite
Schedule: Monday – Friday, 8:00 AM – 4:30 PM
Hours: 40 hours per week
Target Start Date: May 5, 2025
Requirements:
• Licensure: Active Registered Nurse (RN) license
• Experience: Hospice experience required
• System Knowledge: Must have experience with Homecare Homebase (HCHB)
• Strong communication and organizational skills
• Ability to work independently and within a team
Position Summary:
We are seeking a dedicated Registered Nurse for a temporary 8-week assignment to provide compassionate, comprehensive care in a community-based or small facility setting. This role focuses on promoting health maintenance, disease prevention, and case management for individuals with acute, chronic, or terminal conditions. The RN will play a vital role in executing and overseeing care plans, supervising staff as needed, and ensuring compliance with healthcare protocols.
Key Responsibilities:
• Assess, plan, implement, and evaluate individualized nursing care for persons served.
• Collaborate with interdisciplinary teams to develop and implement health programs.
• Coordinate communication between healthcare providers and support staff.
• Manage medication protocols including ordering, reviewing, monitoring, and disposal.
• Document changes in health status and ensure timely physician updates.
• Maintain accurate nursing documentation (daily, monthly, quarterly, annually).
• Train and supervise LPNs/LVNs and unlicensed staff on health-related topics.
• Schedule and follow up on health-related appointments and outcomes.
• Ensure proper infection control measures are followed.
• Conduct site visits (minimum of 5 per day) for various patient needs including admissions and PRNs
