Registered Nurse - Office of Community Care & Patient Coordination Team

What We're Looking For

Experience: Minimum of 2 years of direct patient care within the last 3 years.

Education: Graduate of an ACEN or CCNE accredited nursing program.

Licensure: Active, unrestricted RN license in any U.S. State, Territory, or the District of Columbia.

Certification: Current BLS certification (American Heart Association preferred).

What You'll Do

As an Office of Community Care RN, you will:

• Conduct clinical reviews of consults/requests for completeness and appropriateness.


• Determine care disposition based on utilization review and VA guidelines.


• Review incoming medical records and communicate with ordering providers and PCPs.


• Coordinate care between veterans, VA providers, and community providers.

As a Referral Coordination Team RN, you will:

• Triage specialty consults for clinical appropriateness & completeness.


• Educate veterans on their healthcare options, ensuring informed decisions.


• Serve as a clinical resource for multidisciplinary teams.


• Collaborate with VA specialists to refine consult processes and improve patient outcomes.
