PhyxUp Health is a digital health company building a scalable, data-informed rehabilitation care model. We partner with physical therapy clinics to deliver fully managed Remote Therapeutic Monitoring (RTM). We are seeking a strong clinician who can think critically, interpret patient data, and contribute meaningful clinical insight.

Position Highlights
• 100% remote
• Flexible PRN schedule
• Must be licensed in at least one of the following states: NY
• Work within a digital, data-informed care model

Key Responsibilities
• Review RTM data and patient-reported outcomes
• Identify patients at risk for non-adherence or clinical decline
• Conduct focused check-in calls with clear clinical intent
• Provide concise, actionable summaries to the treating PT
• Suggest care plan considerations based on patient response and progress
• Flag patients who require escalation or closer monitoring
• Document all interactions accurately and in a timely manner

Who You Are
• An experienced clinician comfortable making independent observations
• Strong in communication and clinical reasoning

• Comfortable using digital tools and remote platforms
• Interested in the future of hybrid and remote care

Required Qualifications
• Active PT license in at least one of the following states: NY
• 2+ years of clinical experience in outpatient or rehabilitation settings
• Strong documentation skills
• Reliable internet and professional remote work setup
• Ability to work independently while collaborating with a clinical team

Preferred Qualifications
• Additional state licensure
• Experience with telehealth or digital health
• Interest in data-informed care models
• Availability during standard business hours (9 am – 6 pm), with flexibility to support occasional evening or weekend patient needs
