Job Description:
Peer Support Specialist
Scope: The Peer Specialist (PS) is an active member of Youth Homeless
Demonstration Project (YHDP) and provides peer support to young adults needing
assistance with housing, education, employment, and social-emotional well-being
services.
The PS performs a wide range of tasks to assist peers ages 18-24, in regaining
independence within the community and mastery of life skills. Peer Specialist has a lived experience with homelessness and can speak to their own experience in
supporting young adults.
Reports to: YHDP Case Manager
Responsibilities of a Peer Support Specialist include but are not limited to:

● Collaborate with case manager to create, monitor, and assist clients with their
individual success plans.
● Help clients obtain hiring documents, including IDs, Social Security Cards, and
Birth Certificates.
● Help the client access community resources.
● Work w/ client to navigate public transportation if applicable.
● Assist clients in articulating personal goals through the use of one-on-one and group
sessions.
● Assist YAs in working with their case manager in determining the steps he/she needs
to take in order to achieve these goals.
● Assist YAs in setting up and sustaining self-help (web of support) groups.
● Work with all staff to record services and case management notes.
● Serve as a source of support through regular check-ins.
● Assist in obtaining services that suit that individual’s needs by providing names of
staff, community resources and groups that may be useful.

$23,000-$25,000
