Focus Care Inc.
Location: Various locations in Maine (Travel required)
Compensation: Earn up to $2,000 to $3,200 each week (Average 5-8 visits daily, base rate of $80 per completed assessment, with opportunities to earn additional income for each screening conducted.

Are you a dedicated Nurse Practitioner seeking a flexible role where you can make a meaningful impact on patients' lives? Focus Care Inc. is looking for experienced Nurse Practitioner to join our team in conducting in-home health assessments. This per diem position offers you the chance to work independently, with compensation for each assessment, plus mileage reimbursement.
As a Nurse Practitioner with Focus Care, you will:
⦁ Conduct comprehensive health assessments in patients' homes, including medical history reviews, physical exams, and screenings for chronic conditions.
⦁ Provide education on medication management, disease processes, and preventive care.
⦁ Identify and document diagnoses to support care management and active treatment.
⦁ Collaborate with primary care providers to communicate findings and address potential gaps in patient care.
⦁ Flexibly schedule your appointments throughout the week, with no on-call, weekends, or holidays required.
Minimum Requirements:
⦁ Completion of at least 30 in-home assessments per month.
⦁ Valid Nurse Practitioner license in the state of Maine.
⦁ Board Certification from ANCC or AANP
⦁ Minimum of 1 year of clinical experience; experience with geriatric populations preferred.
⦁ Strong communication skills and a commitment to delivering compassionate care.
⦁ Reliable transportation for travel within the assigned areas.
⦁ Ability to manage various dwelling conditions and engage with patients comfortably in their home environments.
