Job Summary: The Nurse Case Manager is responsible for coordinating patient care across various settings to ensure optimal outcomes and efficient use of healthcare resources. This role involves assessing patient needs, developing and implementing care plans, and collaborating with healthcare professionals and community resources to support patients throughout their care journey.

Key Responsibilities:

Patient Assessment: Conduct comprehensive assessments of patients’ physical, emotional, and social needs. Identify high-risk patients and those requiring complex care coordination.
Care Planning: Develop individualized care plans in collaboration with patients, families, and healthcare providers. Set goals, establish interventions, and coordinate resources to achieve optimal health outcomes.
Care Coordination: Facilitate communication and coordination among healthcare providers, specialists, and community resources. Ensure that patients receive timely and appropriate care and follow-up services.
Patient Education: Educate patients and their families about their diagnoses, treatment options, and self-management strategies. Provide guidance and resources to help patients navigate their care and make informed decisions.
Resource Management: Monitor and manage the utilization of healthcare resources, including referrals, diagnostic tests, and treatments. Advocate for patients’ needs and work to minimize unnecessary or duplicative services.
Case Documentation: Maintain accurate and up-to-date patient records, including care plans, progress notes, and communications. Ensure compliance with regulatory and documentation requirements.
Quality Improvement: Participate in quality improvement initiatives to enhance care processes, patient satisfaction, and health outcomes. Review and analyze patient data to identify trends and areas for improvement.
Discharge Planning: Plan and coordinate patient discharge processes, ensuring a smooth transition to home or other care settings. Provide post-discharge follow-up and support to prevent readmissions and complications.
Job Requirements:
Qualifications:

Education: Bachelor’s degree in Nursing (BSN) required; Master’s degree in Nursing or Healthcare Administration preferred.
Experience: Minimum of 3 years of nursing experience, with at least 1-2 years in case management or a related field.
Certification: Active and unrestricted RN license in [State/Country]. Certification as a Case Manager (CCM) or equivalent is highly desirable.
Skills: Strong clinical assessment and critical thinking skills, excellent communication and interpersonal abilities, and proficiency in case management software and electronic health records (EHR).
Attributes: Detail-oriented, organized, and capable of managing multiple priorities. Demonstrated ability to work collaboratively in a team-oriented environment and to advocate for patients’ needs.
