Job Overview

We are seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) or LMSW to join our dynamic healthcare team. In this vital role, you will provide comprehensive social work services to diverse patient populations, including children, adults, and geriatric individuals. Responsibilities

Conduct thorough patient assessments to identify psychosocial needs, including mental health status, family dynamics, and community resource requirements.
Develop individualized care plans that incorporate behavioral therapy, crisis intervention, and end-of-life care strategies aligned with clinical ethical guidelines and HIPAA regulations.
Provide clinical counseling and psychotherapy to patients and families dealing with issues such as addiction, grief, dementia, PTSD, or developmental disabilities.
Coordinate community resources and facilitate discharge planning to ensure seamless transitions from hospital or inpatient settings to home or assisted living environments.
Document all patient interactions accurately within electronic medical records (EMR) systems while adhering to medical documentation standards and ICD coding protocols.
Collaborate with interdisciplinary teams—including medical professionals, nurses, therapists, and case managers—to optimize patient care outcomes through clinical interprofessional collaboration.
Engage in clinical research activities when applicable, contributing data collection efforts that support ongoing quality improvement initiatives.
Qualifications

Master’s degree in Social Work from an accredited program; valid licensure as a Licensed Clinical Social Worker (LCSW).
Proven experience working with diverse populations across settings such as hospitals, nursing homes, outpatient clinics, or hospice care.
Strong knowledge of social work practice standards, medical terminology, HIPAA compliance, and clinical ethical guidelines.
Demonstrated expertise in child & family counseling, behavioral health interventions, crisis management, and group therapy modalities.
Familiarity with EMR systems, CPT coding for billing purposes, ICD classifications (ICD-9/10), and Medicare regulations.
Ability to handle sensitive situations with empathy while maintaining professionalism under pressure.
Excellent client communication skills coupled with the ability to work effectively within multidisciplinary teams.
