Position Overview
The Hospice RN works collaboratively with an interdisciplinary care team to develop and manage individualized hospice care plans, ensuring patient comfort, dignity, and quality of life. This role requires strong clinical judgment, excellent time-management skills, and the ability to work independently in the field.
Key Responsibilities
Collaborate with the interdisciplinary team to develop and implement patient-centered care plans
Provide compassionate hospice nursing care within the assigned service area
Ensure comfort, symptom management, and emotional support for patients and families
Complete accurate and timely documentation to meet regulatory and organizational deadlines
Independently manage schedule and workload during overnight shifts
Utilize tablets, mobile devices, and EMR systems for documentation and communication
Qualifications
Active Registered Nurse (RN) license in the state of practice
Prior nursing experience required; hospice or home health experience preferred
Proficiency with EMR systems and mobile technology
Strong communication, organizational, and clinical skills
Ability to work independently in the field

Full-time overnight schedule
Monday – Thursday | 5:00 PM – 8:00 AM
