Duties

Assist patients with Activities of Daily Living (ADLs) such as bathing, dressing, grooming, and toileting to ensure their comfort and dignity
Monitor patient health by observing vital signs, reporting changes, and maintaining accurate medical documentation using Electronic Health Record (EHR) or Electronic Medical Record (EMR) systems
Support medication administration following proper protocols and ensuring compliance with HIPAA regulations for patient privacy
Provide specialized care for individuals with dementia, Alzheimer’s disease, or other memory impairments through tailored memory care techniques
Operate medical equipment such as Hoyer lifts and perform catheter care safely and effectively
Assist with mobility using assistive devices like wheelchairs or Hoyer lifts to prevent falls and promote independence
Prepare nutritious meals and assist with feeding while adhering to dietary restrictions and safety standards
Perform light housekeeping tasks including cleaning, laundry, and maintaining a safe environment in accordance with home & community care best practices
Document all patient interactions thoroughly in medical records and ensure compliance with healthcare regulations
Job Requirements:
Qualifications

Proven experience in home health care, assisted living facilities, nursing homes, or long-term care settings
Strong knowledge of medical terminology, vital signs measurement, medication administration, and patient observation techniques
Certification in CPR (Cardiopulmonary Resuscitation) is required; additional certifications such as hospice care, dementia care, or Alzheimer's care are highly valued
Experience working with people with developmental disabilities or disabilities requiring specialized support is preferred
Familiarity with EMR/EHR systems for accurate medical documentation and record keeping
Ability to work independently while maintaining excellent communication with healthcare teams and family members
Compassionate attitude with a commitment to providing respectful, patient-centered care
