Job Requirements:
Key Responsibilities
Provide health education, outreach, and non-clinical support to clients
Assist clients in navigating healthcare, social services, and community resources
Coordinate appointments, referrals, and follow-up support
Offer emotional support, advocacy, and companionship
Collaborate with healthcare teams and community partners to ensure continuity of care
Maintain accurate client records, documentation, and service notes
Qualifications
Experience in community outreach, social services, healthcare support, or non-profit work preferred
Experience supporting seniors, vulnerable populations, or individuals with disabilities is an asset
Strong communication, interpersonal, and organizational skills
Compassionate, culturally sensitive, and client-focused approach
