Job Description:
As a Case Manager, you will provide oversight for the Independent Living (ILS) and Tailored Day programs. You will supervise employees and ensure that they have the necessary support to help clients achieve their goals.
Job Requirements:
• Provide oversight of Independent Living and Tailored Day programs.
• Responsible for the hiring, training, monitoring, supervision, coaching, and evaluation of employees, establishing clear expectations and fostering a culture of compliance and accountability.
• Drive service utilization through staff engagement.
• Ensure accuracy and timely submission of payroll and billing data.
• Ensure quality services by monitoring and ensuring delivery of services in accordance with the ID Team. Utilize customer service and problem-solving skills as needed to resolve issues.
• Process referrals and facilitate matching new clients with current staff members.
• Complete new client assessments and intakes.
• Maintain client charts and authorizations.
• Carry a caseload and provide direct client services as needed.
• Coordinate group social activities by planning and leading activities.
• Understand current local, state and federal law and regulations and apply knowledge to ensure compliance.
• Participate in outreach to educate the community about available services.
