Position Overview: Bi-Lingual (Spanish) Care Manager / Intake Specialist
*This is a hybrid position – you will be responsible for both Case Management and Intake Submission.
Location: New York City (must reside within the 5 boroughs)

Seeking an experienced, compassionate, detail-oriented Bilingual Case Manager/Intake specialist to join our team. Our mission is to support individuals, and families by connecting them with the services and resources they need to thrive.
Case Management:
You will provide care coordination and support to individuals and families, ensuring their needs are met through individualized service plans. You will work closely with community providers, schools, and healthcare professionals to build a strong support network.
Responsibilities include:
Conduct intake assessments and develop individualized care plans
Provide ongoing case management, advocacy, and support to families
Coordinate services across medical, behavioral, educational, and social domains
Monitor progress and adjust care plans as needed
Maintain accurate, timely documentation and reports
Participate in regular supervision and team meetings
Intake Submission:
You will assist with intake submissions for new clients entering the program. This includes collecting, reviewing, and submitting intake documentation in compliance with program standards.
Responsibilities include:
Completing intake forms and required documentation
Verifying client eligibility and ensuring accuracy of submitted information
Coordinating with families and referral sources during the intake process
Submitting intake packets in a timely and organized manner.
Preferred Qualifications:
Experience working with children, youth, or families in social service, behavioral health, or healthcare settings
Compensation:
$30 per hour
