We are seeking a compassionate and dedicated Hospice Registered Nurse (RN) to join our team. This full-time overnight role focuses on providing high-quality, patient-centered end-of-life care in a supportive and collaborative environment.

Position Overview
The Hospice RN partners with an interdisciplinary care team to develop and manage individualized hospice care plans, ensuring comfort, dignity, and quality of life for patients. This role requires independent clinical judgment, strong time-management skills, and hospice experience.

Key Responsibilities

Collaborate with the interdisciplinary care team to develop and deliver patient-centered care plans
Provide compassionate hospice nursing care within the assigned service area
Ensure symptom management and comfort-focused care
Maintain accurate and timely clinical documentation to meet regulatory deadlines
Independently manage schedule and workload during overnight shifts
Utilize tablets, mobile devices, and EMR/EHR systems for documentation and communication

Required Qualifications

Active Registered Nurse (RN) license in the state of Pennsylvania
Associate degree or higher in Nursing
2+ years of hospice nursing experience required
1+ year of experience using Electronic Medical Record systems, including:

Electronic Medical Record (EMR)
Other EMR platforms
TruChart EHR

Strong clinical judgment, communication, and organizational skills
Ability to work independently in a hospice/home-based setting

Schedule & Shift Details

Full-time overnight schedule
Monday – Thursday | 5:00 PM – 8:00 AM

Travel Requirements

Local travel required within the assigned service area
