Admissions Coordinator • Prepare daily and monthly census reports. • Verify insurance benefits for all new referrals and readmission residents (Medicare, Medicaid, private, HMO). • Check Medicare coverage on all residents monthly (up to 110 residents). • Provide tours of facility. • Prepare rooms for new admissions. • Attend and schedule multiple marketing and networking events monthly. • Advise doctors of new admissions they are assigned to. • Assist residents in completing admission and readmission paperwork. • Staff new referrals with the Director of Nursing and Administrator. • Serve residents lunch on a weekly basis. • Ensure other facilities provide accurate 3008 and Pasrr documents before accepting admissions. • Update face sheets for charts as needed. • Obtain credentialing documents from physicians. • Work with social workers and discharge planners to coordinate admissions from nursing homes and hospitals. • Set up admissions for patients discharged from home and assisted living facilities.

Care Manager • Visited the elderly and disabled in their homes, in assisted living facilities, in hospitals, and in nursing homes to assess medical needs. • Coordinate in home services and alternative placement for seniors needing moderate to maximum assistance with activities of daily living. • Worked closely with Department of Elder Affairs to ensure that members were receiving adequate services based on their medical conditions and personal needs. • Corresponded with Department of Children and Families to assist members with maintaining Medicaid eligibility. • Handled complaints from members and their families while working with contracted providers to resolve them. • Participated in local community events informing visitors about the program and how it is funded by the government to delay or avoid nursing home placement.

Victim Advocate • Accompanied and assisted clients with court appearances and legal appointments. • Helped victims file for relocation assistance and injunctions. • Participated in the Sexual Assault and Domestic Violence Task Forces. • Co-facilitated the Domestic Violence Support Group. • Core Competency Advocacy Training Certified as of 10/7/2005. •Completed over 87 hours of domestic violence training during first year of employment. •Answered Domestic Violence Hotline calls and assisted with supervising the Domestic Violence Shelter during evenings and weekends.
