Job Summary:
Position: Licensed Practical Nurse
Schedule Available: Sunday-Wednesday 9a-5p, every other Thursday and Friday 5p-10:30p, and every other Saturday 9a-5p
Pay: Based on Experience
* Health Insurance Available to employees who work 32 hours a week or 130 hours average in a month. We offer: Health, Dental, Vision, Aflac.
Patient Care required:
LPN to provide medication management and administration
LPN to provide incontinence care
LPN to provide supervision of safety assurance
LPN to provide all personal care
LPN to complete transfers (Hoyer Lift)
LPN to provide G-tube feedings, care and maintenance
LPN to provide Meal Prep if Needed
LPN to communicate regularly with RN case manager and PCP as needed.
Experience: 1 year of Nursing preferably in the field of Home Health preferred
To be considered for this position, candidates must have the following:
LPN License
CPR (BLS Provider)
2 Step PPD
2 Forms of I.D
Medical Exam
Valid Driver's License
Reliable Transportation
