JOB LOCATION: Position is primarily based in Sunbury, PA but requires travel throughout Northumberland County.
ORGANIZATIONAL RELATIONSHIP: The Early Head Start LPN / Health Monitor reports directly to the Early Head Start Supervisor and in his/her absence to the Head Start Programs Manager.
JOB DEFINITION: The Early Head Start LPN / Health Monitorís major responsibility is to facilitate relationships between the home, the parents and community health agencies to support a healthy lifestyle and environment.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Establish and maintain health records for individual children and pregnant women.
Provide Health Education for children and families and pregnant women enrolled in the EHS program.
Request reports from doctors, dentists, specialists and others when necessary.
Attend Team Meetings.
Make regular home visits.
Provide prenatal information.
Schedule, coordinate and implement sensory and health screening for children.
Help facilitate the bi-annual Health Advisory Committee Meeting (HAC).
Coordinate transportation for Health Services.
Assume responsibility for the implementation and continued development of the Health Component as required by the Head Start Program Performance Standards.
Attend regional and statewide trainings and workshops.
Maintain current physical on a biennial schedule.
NON-ESSENTIAL DUTIES AND RESPONSIBILITIES: Performs other related duties as assigned.
EDUCATION, EXPERIENCE, AND SKILLS REQUIRED:
Requires minimum of a High School Diploma or equivalent General Education Diploma (GED).
A valid Pennsylvania Licensed Practical Nursing Certification.
Requires experience working with young children and families.
Excellent communication and interpersonal skills.
Ability to accept great cultural diversity, interest and concern for children and families.
Flexibility to accommodate a changing caseload, ability to cope with stressful situations, and a professional attitude are required.
ADDITIONAL ELIGIBILITY QUALIFICATIONS:
Current, approved Act 34 (PA State Police), Act 151 (PA Child Abuse Registry) and Act 114-FBI Fingerprint clearances are required and must be renewed at appropriate, specific intervals pursuant to Act 153 of 2014 (revised Act 15 of 2015).
Current, approved Act 126 Mandated Reporter Training certificate showing completion of a minimum of three (3) hours training every five (5) years.
PROFESSIONAL DEVELOPMENT REQUIREMENTS:
Accumulate mandated continuing education credits.
Attend training seminars and in-service programs identified as important to the operation of the program.
Continued training in First Aid is required.
Sedentary: Able to sit for extended periods of time, occasional walking, standing (as necessary).
Must have strength and ability to lift young children (approximately 45 lbs.).
Must have strength and ability to carry teaching materials/bags (approximately 20 lbs.).
Dexterity: Finger movements (computer keyboard, calculator), eye-hand coordination (office equipment, etc.).
Audio-Visual Requirements: Within normal range with or without correction.
Is able to move throughout all buildings, floors and grounds.
Worksite Environment: Primarily indoors-office setting with occasional exposure to outdoor elements/climate during the day.
Mental demands: Steady response to unpredictable situations, ability to remain calm and productive during stressful situations. Demonstrates independent judgment, discretion and is able to manage multiple projects and deadlines.
Must be able to assist children with self-help skills.
Must be able to arrive on schedule at various locations over a wide geographic area.
Must be able to negotiate stairs and rough terrain at centers and homes.
Ability to travel to various work site locations and meetings as needed.
TIME AND SITE REQUIREMENTS:
This is a full time non-exempt position working 7.5 hours per day, 260 days per year. Additional working hours must be pre-approved by immediate supervisor.
Position require attending evening meetings and overnight trips.
Attendance at work is a requirement of this position; any and all absences must be approved by the immediate supervisor.
