What You’ll Do
Conduct In‑Home Assessments: Perform comprehensive health evaluations including medical history review, physical Nurse Practitioner – In-Home Health Assessments

Compensation: $2,000–$3,200/week (Base rate of $80 per completed assessment + bonuses for additional screenings)

Join Focus Care Inc. as a per diem provider conducting in-home health assessments across Maine. This role allows you to work independently, enjoy flexible scheduling, and earn competitive pay while improving patient outcomes.
What You'll Do:
⦁ Perform comprehensive in-home health assessments, including:
⦁ Medical history reviews
⦁ Physical examinations
⦁ Screenings for chronic conditions
⦁ Educate patients on medication management, disease prevention, and health maintenance
⦁ Identify and document diagnoses to support care management and treatment planning
⦁ Communicate findings with primary care providers to ensure continuity of care
⦁ Schedule appointments at your convenience — no on-call, weekends, or holidays
Minimum Qualifications:
⦁ Active Nurse Practitioner (NP) license in the state of Maine
⦁ Board Certification through ANCC or AANP
⦁ At least 1 year of clinical experience (geriatrics preferred)
⦁ Must complete a minimum of 30 in-home assessments per month
⦁ Excellent communication and interpersonal skills
⦁ Reliable transportation and comfort working in varied home settings
Compensation & Benefits:
⦁ Earn $2,000–$3,200 weekly based on average of 5–8 visits/day
⦁ Base rate: $80 per completed assessment, plus additional earnings per screening
⦁ $500 Sign-On Bonus after completing 30 assessments
⦁ Mileage reimbursement
