Description:
This is a full-time position responsible for individualized program planning for adults with disabilities served by Greene Arc at its Vocational Training Center in Ruff Creek, PA, and at licensed community homes throughout Greene County.
Responsibilities include:
The individual in this position will develop and implement person-centered plans designed to promote employment and independence. Responsibilities include coordination or completion of assessments; coordination or development of individual program plans; coordination of training and development for individuals; development of planned program activities; participation in interdisciplinary team meetings; and other duties as assigned. These duties may involve Direct Care of individuals when needed, in the program areas.
The person will also provide guidance and support to direct care staff in the implementation of goals and objectives, record keeping, community participation, individual’s direct care as well as communication with parents, families, and County Human Service Programs.

Job Requirements:
Minimum Qualifications:
Accredited Bachelor’s degree and one-year work experience working directly with persons with intellectual disabilities.
OR
An associate’s degree or 60 credit hours from an accredited college or university and 2 years work experience working directly with persons with intellectual disabilities.
Strong documentation and computer skills required.
Valid Driver’s License and Clean Driving Record.
Must Pass Child Abuse History, FBI and Criminal Record Check Clearances.
