Job Description:
We are looking for a “Community service worker ” for BeConnected Support Services LTD. who can perform the following duties:
• Prepare reports or case histories
• Appraise clients' needs or eligibility for specific c services
• Implement life skills workshops
• Assess client's relevant skill strengths and development needs
• Conduct follow-up assessments
• Develop, coordinate and implement the delivery of specific services within the community
• Provide suicide and crisis intervention
• Supervise activities of clients living in group homes and halfway houses
• Conduct individual and group counseling sessions
• Liaise with other social services agencies and health care providers involved with clients
• Assist in evaluating the effectiveness of treatment programs
• Accompany clients to appointments and social outings
• Interview clients, prepare case histories, and assess problems

Job Requirements:
