Job Description:
Maximus is hiring a Nurse Practitioner to conduct Person-Centered assessments in accordance with State directives, Maximus processes, and regulatory timeframes, which includes: Reviewing records for medical and mental health history/diagnoses, gathering relevant information, completing a bio-psychosocial assessment and brief neuro-mental exam with the individual.
This is a hybrid remote opportunity located within the state of California with some travel required to complete face to face assessments. Candidates must reside in and be able to travel throughout specific regions of the state of California.

Job Requirements:
Essential Duties and Responsibilities:
Responsible for completing timely and quality assessments per working day in Medicaid certified facilities, such as a Skilled Nursing Facility (SNF), Acute Care Hospitals (GACH), or STP (long term psychiatric facility), with individuals diagnosed with or suspicion of a mental illness or intellectual/developmental disabilities
Conduct on-site and remote (using HIPPA compliant Zoom telehealth) long-term care assessments to determine an individuals needs for long-term services and supports
Ability to work flexible schedule, if desired, including evenings, weekends and holidays
Ability to travel by car as required by client contract
Minimum Requirements:
Master's Degree
Required licensure, can be any of the following: Licensed Psychiatrist; Clinical Psychologist; Clinical Social Workers; Marriage & Family Therapist; Professional Clinical Counselor; Nurse Practitioner.
Position requires associated licensure in the state of California
Must have three (3) years out of the past five (5) years experience with providing direct patient care for individuals presenting with a serious mental illness and with providing direct care for patients.
At least five (5) years of clinical experience
Two (2) years performing long-term care clinical assessments with individuals diagnosed with SMI (Serious Mental Illness)
Working knowledge of community support programs.
Perform all job duties in compliance with Person First standards, HIPAA guidelines, and company confidentiality policies and procedures
Working knowledge and understanding of medical diagnoses and prescribed medications
Ability to multitask while effectively performing job duties, which include prioritizing tasks; ensure all voice mail and email messages are monitored and responded to within 24 hours of receipt or as required by client contract
Ability to work independently, as well as within a team while exhibiting excellent interpersonal skills
Strong written and verbal communication skills
Ability to collect data, define problems, establish facts, and draw valid conclusions for supporting a superior written rationale for review determination
Knowledge and training with computer skills, word processing and email (Microsoft Word, Outlook preferred), ability to work securely in remote locations, research information on the internet, utilize company resources/ tools stored on company sites
