Job Description:
Duties will include:
* Individual case management
* Assist with crisis interventions
* Provide life skills and/or individual rehab services
* Accompany and provide support to clients when accessing services in the community (as needed)
* Maintain and update client files and documentation in accordance with DHS guidelines
* Maintain and submit records in a timely manner
* Participate, coordinate and implement cooperative arrangements and/or team conferences with various inter-disciplinary team members in the development and implementation of client treatment and aftercare plans
* Coordinate services with various public and private community health, mental health, and social service agencies to ensure appropriate services for clients.
* Transport client(s) to connect with necessary community resources
Job Requirements:
BA and/or 2 yrs mental health experience preferred
Experience in working with the homeless population preferred
CA Driver's License Required
Full COVID 19 Vaccination Required
