Description:
Reporting to the Director of Shelter Services, the Respite Coordinator/Case Manager is responsible for facilitating and managing a program providing short-term, residential, post-medical care for homeless persons who are not ill enough to remain hospitalized, but who are too frail to be homeless. This position handles all Respite Program functions by marketing the program; and conducting outreach to hospitals and other appropriate sources to obtain referrals. The Coordinator also conducts screenings and manages and referrals and applications; conducts intakes; ensures that respite clients receive appropriate assessment; treatment plans and case management services to enable them to move towards the levels of independent living they are able to achieve.
Duties:
ESSENTIAL FUNCTIONS AND KEY ACCOUNTABILITIES Responsible for: 1.Promoting Community Awareness of the program to potential referral sources by describing its services and successes;
2.Conducting outreach to potential referrals sources to obtain clients for Respite Program and achieving and maintaining appropriate utilization goals;
3.Ensuring that potential clients are screened;
5.Conducting intakes;
6.Assessing client and identifying service needs;
7.Ensuring that clients have appropriate articles needed upon entry into the program (such as linen, towels needs upon entering the Respite Program;
8.Ensuring that service plans appropriately identify client needs and identifies the levels of health and independence the client can achieve;
9.Provide client service needs through case management and referrals services;
10.Provide other assistance to clients as needed;
11.Developing and maintaining client medical and case management reports in compliance with confidentiality and HIPAA requirements;
12.Preparing program reports as required; and
13.Performing other duties as assigned.
Qualifications:
REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES
1. Generalized background with broad knowledge of residential care for persons with chronic illness;
2. Knowledge of behavioral health practices and procedures; psychosocial rehabilitation; psycho-education; chemical dependency, motivational enhancement strategies; assessment, crisis intervention, process facilitation and supportive counseling and case management activities.
3. Must be able to learn the Multicultural Relational Approach and Cognitive Behavioral Therapy (CBT) models.
4. Must have excellent critical thinking and problem-solving skills;
5. Knowledge of proven marketing and program outreach techniques;
6. Knowledge of Central Coast hospitals and other potential referrals sources.
7. Experience working with multiple types of service programs.
8. Exhibit good judgment, practical day-to-day experience working with a diverse group of people, and hardworking.
9. Extensive experience and judgment to plan and accomplish goals. Creativity and latitude.
10. Strong written and verbal skills, organizational excellence, and team-oriented.
11. Ability to handle confidential information with discretion and integrity.
12. Proficient in Microsoft Word, Excel, and Power Point.
Education and Experience Requirement(s) Bachelors degree in behavioral health or related field; and at least one year of supervisory experience; or an appropriate equivalent of the above requirements. Registered Nurse or Licensed Vocational Nurse
Ability to work independently and within a team environment and exercise mature judgment
Must have good computer skills and strong written and verbal communication skills
Must pass a background check, and must have a valid driver’s license and clean driving record. Must pass a pre-employment drug screening according to federal guidelines.
CPR/First Aid certification (on-site training) and TB Testing
Working Conditions General residential environment; Extensive field work is necessary; and must be able to respond to calls.
Certifications & Licenses Required/Preferred Valid California drivers License is required with a good motor vehicle record. RN or LVN.
