Provides supportive services to individuals with mental illness regarding management of their home and mental illness. Teaches daily living skills and ensures individual's safety and well-being. Responds to emergency or crisis situations.

ESSENTIAL RESPONSIBILITIES, EXPECTATIONS & COMPETENCIES

• Supports the mission and follows the procedures and policies of Solutions Community Counseling and Recovery Centers.
• Provides emotional support by interacting with client face to face. Ensures progress by monitoring emotional and mental health, treatment plans, progress and other items.
• Assists client and/or family by securing services and resources including housing, medical treatment, financial assistance and transportation.
• Collaborates with client and/or family members or client representatives by discussing or responding to treatment plans and other needs. Administers family or group counseling as needed.
• Advocates for client in community and with other agencies including Human Services, Social Security Administration and, County and Common Pleas Courts.
• Communicates with agency service providers, and other professionals by attending meetings and training sessions.
• Maintains professional and technical knowledge by reviewing current research and practices and attending seminars and training sessions.
• Keeps accurate records in Carelogic by completing and maintaining required documentation including diagnostic assessments and progress notes.
• Adheres to all applicable legal, professional and ethical standards, including confidentiality, in the performance of clinical services.
• Maintains confidentiality of client records and other sensitive subject matter.
• Has the ability to exert control over the use of work-time hours. Completes tasks in a timely manner and meets all deadlines.
• Works assigned schedule, exhibits regular and predictable attendance and works outside or normal schedule to meet workload demands. Performs other related duties as required.

OTHER DUTIES AND RESPONSIBILITIES
Maintains productivity expectation of 52% of paid hours.

Motor vehicle (if applicable); computer; telephone, fax machine, copier and other general office equipment.

Clients; general public; social service agencies; Human Services and other governmental agencies; physicians and other medical providers.

WORKING CONDITIONS
Normal office working conditions. When meeting with clients, verbal de-escalation and may be required in your scope of work. Employee is regularly exposed to outdoor weather conditions and road hazards while traveling to off-site locations.

Employee occasionally travels in a motor vehicle to attend meetings and meet with clients. Employee occasionally lifts varying weights up to 25 pounds when moving files, boxes, equipment or supplies.

REQUIRED KNOWLEDGE, ABILITIES & SKILLS
Knowledge of: mental illness and substance abuse; standard custodial methods, materials, equipment and supplies; basic mathematics.
Ability to: develop and maintain effective working relationships with associates, supervisors, job contacts and general public; maintain confidentiality of non-public and sensitive subject matter; accurately post information to written records; exhibit flexibility in work schedule and job assignments; exercise patience and sound judgment in dealing with persons with mental illness or substance abuse; solve problems; perform light manual labor; lift and carry job related tools and equipment.
Skill in: verbal and written communication; safe operation of motor vehicle; operating general office equipment; preparing written reports; operating household and other job related equipment; performing mathematical calculations including addition, subtraction, multiplication, decimals, percentages and fractions.

LICENSURE OR CERTIFICATION REQUIREMENTS
A Bachelor degree in a related field is required.
