About the Position

This Supervisor/ FTC facilitator will be responsible for managing a team of two case planners and intervenes with families in order to: reduce risk, promote child safety and well-being, and prevent the placement of children into foster care. The responsibilities will also include assessment, coordination of services, counseling, advocacy, recording and maintenance of case records.

FTC Duties & Responsibilities (but not limited to):

Lead conference participants in the group process and become involved in the discussion needed to ensure the best decision in regards to the safety and well-being of the children.
Direct the group process and structure of the conference.
Through the skills of brainstorming, assessment, negotiation and consensus building, create in partnership with families and other participants, a safety plan that reflects the family’s strengths and incorporates community resources and addresses the safety, behavioral, educational, and psychological and permanency needs of children
Facilitates discussion of safety and permanency ensuring that:
No family leaves the FTC table until he/she understands the agency’s concerns in relation to safety, risk and permanency.
Every family member leaves the FTC table with a clear understanding of the steps to take to achieve the desired case outcomes
Information shared in the conference is clear and straightforward and that all present have a turn to convey the issues as they view them.
Effectively manages conflict and strong emotions as they arise by utilizing Conflict Management skills.
Accurately records FTC information and decisions on designated forms and in Connections and provide a copy of the report to all conference participants.
Help the family understand agency’s concerns in relation to safety and risk.
Promotes an inclusive, safe and supportive environment where all participants will be assured that they can and will be heard.

Supervisory Duties & Responsibilities:

Ensures that case planners are making comprehensive family assessments using the Solution Based Casework method and work toward safe parenting, family well-being, restoration of functioning, or maintenance.
Guides case planners to provide parent skill development and coaching, case interventions, case management services in alignment with the Solution Based Casework Family Agreement.
Ensures that case planners make consistent home and school visits, and work in cooperation with other providers to assist the families with behavioral change.
Responsible for assisting the Program Director in carrying out agency and program goals and objectives, which include timely submission of all required reports, timely development of Family Agreement, FASPs, and progress notes with the use of Connections and PROMIS.
Responsible for facilitating Family Team Conferences.
Co-lead or lead group activities and programs as assigned by Program Director.
Responsible for teaching Solution Based Casework and related skills to case planners and providing instruction on clinical skill development
Provide weekly individual supervision to case planners focusing on skill development and case-specific family progress.

Job Requirements:
Desired Qualifications:

· Master’s Degree in social work or related field.

· Bi-lingual (Spanish speaking) preferred.

· Must be certified in SBC within 9 months of hire.

· Critical Competencies: Detail-oriented; Excellent verbal and written communication skills; flexibility, professionalism; ability to function as a team player and possesses a demonstrated passion for advancing the work of serving children and families.
