Case Manager / Patient Care Attendant Supervisor
Manage the care of terminally ill patients in a home setting. This included scheduling/transporting patient to/from all medical appointments/procedures, managing medications, patient/family education regarding medical conditions, develop/implement care plans, meet nutritional needs to ensure optimal weight, and assist patient with physical and occupational therapy. It was also my responsibility to supervise 5-6 patient care attendants to assure continuity of care and patient safety. In addition, I assisted the family with the grieving process by locating and referring to outside sources such as grief counseling.
