Objective:

Experience:
Responsible for clinical case management for children placed in foster care with social/emotional/behavioral
and/or developmental disabilities. Develop/implement treatment plans, coordinate services with treatment team. Write & process monthly & quarterly reports, ensure compliance with all licensing/regulations. Transport children to therapy/state agency. Assume on-call duty for crisis intervention/intake assessment. Conduct home/school/court & monitor visits.
Facilitate Homeless Veterans Program w/mental/social health/incarceration/court order mandated employment assessment/treatment plans/referrals, class/job training workshops/job training. Conduct testing/evaluations, provide vocational counseling/job assistance. Maintain documentation case files, placements of clients, develop work experience placements in the community. Perform community outreach, schedule employer interviews for clients/follow up. Attend staff meetings, job placement network conferences/trainings. Provide client budget analysis/counseling as needed. Refer to agencies for assistance. Develop exit plans/transition, housing/follow-up clients placed in jobs/screening for updated files/reports.

National University, San Diego, CA - Master of Arts in Human Behavior/Counseling
National University, San Diego, CA - Bachelor of Science in Organizational Behavior/Management
Grossmont College, El Cajon, CA - Associate of Science in Computer Applications
