Job Description:
Blessed Healthcare and Staffing Agency is looking for an RN Case Manager
Some Responsibilites include:
Making visits when necessary to patient's homes.
Provides skilled nursing care to patients requiring professional nursing services in their homes.
Observes patients for changes in condition, reports changes appropriately to the physician, Administrator/Clinical Director, and other members of the home care team.
Initiates and maintains implementation of orders for medication and treatments as prescribed by the physician.
Initiates and applies appropriate preventative, therapeutic, and rehabilitative procedures and techniques.
Assists and instructs patients, family members or other patient representatives in the implementation of the care plan necessary to promote the patient's health, safety, and independent living.
Participates in case conferences with other members of the health care team.
Participates in quality improvement/ assurance activities as assigned.
